How to rate an item

I am encouraging each customer to rate and provide feedback for my work and my support, but many of them don’t know how to do it. Further more, some customer that seem to have experience using computers, websites and so on, he found the rating form quite hard he said.

Also the wiki does not show any instruction on that.

Perhaps some author who also purchased other items can help? A screenshot would be perfect.
