Just change the name of your practice, the address and other info – and you’re good to go. Font used: Relay Medium (http://www.fonts.com/FindFonts/Detail.htm?pid=409743&/cgi-bin/MsmGo.exe?grab_id=0&page_id=6650&query=RELAY&SCOPE=Fonts) but you can change fonts and use whatever you like.
