With 3D rendered image mask – easy to modify, just open Smart Object (layer marked in Red) – double click or right click and choose “enlace Contents”, place your image and save. Same with the main header text – marked in yellow, double click or right click, Edit Contents, retype, save.

If it asks for a displacement map, it is a .psd file included in this package, called displacement_glass.psd – just point photoshop to it.
